What We Do
As a support coordination agency, Amazing Strides helps adults with intellectual and developmental disabilities reach their full potential and live their best lives. We partner with clients, families and caregivers to identify strengths and create a personalized service plan to open a world of possibility and opportunity for clients. Then we connect them with local support providers and resources—and follow up regularly to ensure their needs are met.
Who We Serve
To get support through the N.J. Division of Developmental Disabilities (DDD), you must:
- Live in New Jersey.
- Qualify for Medicaid.
- Be at least age 21.
- Have a developmental disability.
- Have a chronic, lifelong physical and/or intellectual impairment that began before age 22 and substantially limits you in at least three of these activities: self-care; learning; mobility; communication; self-direction; economic self-sufficiency; independent living.

How We Work
Here’s our simple, four-step process for getting you the support you need:

1. Choose an agency
Once you’re eligible for (or already receive) DDD services, you can request a support coordination agency or have one assigned automatically.

2. Get started
Once Amazing Strides is selected, our executive director will contact you and/or your caregivers to discuss your needs.

3. Develop your plan
We’ll assign a permanent support coordinator, who will meet with you and/or your caregivers to create your Individual Service Plan (ISP).

4. Get your services
Your support coordinator will ensure you receive your available services through DDD—and they’re always available if you need help or have questions.
