We offer Pre-Tenancy and Tenancy Sustaining Housing Support Services..

to help individuals secure and successfully maintain safe, stable housing within their communities.

Our Housing Supports include

Pre-Tenancy Support

Housing search assistance, application support, gathering documentation, landlord outreach, and move-in coordination.

Tenancy Sustaining Support

Ongoing stability support, landlord relationship management, connection to community resources, and eviction prevention strategies.

Move-In Supports

Assistance with coordinating approved move-in supports, including security deposits, realtor fees, furniture, household essentials, groceries, and moving-related expenses through Medicaid Managed Care Organizations.

These services are designed for Medicaid-eligible adults who are currently homeless or at risk of losing their housing, providing the tools and advocacy needed to live safely and independently.

Who can complete the form?

What you’ll need

You (the member), a family member or guardian, a trusted friend, or a staff person assisting you.

Your Medicaid information, your MCO name/ID (if available), your county, and a brief checklist about your current housing situation and support needs.

If you prefer, you may also ask your MCO to place a referral for the Housing Supports Program and confirm eligibility.
